UTILIZAÇÃO DE ENXERTO DE CARTILAGEM E RETALHO MIOCUTÂNEO DE TRIPIER PARA RECONSTRUÇÃO DE PÁLPEBRA INFERIOR: UM RELATO DE CASO

Abstract

INTRODUCTION: The lower eyelid is a fundamental structure for protecting the eyeball and, acting as an adjuvant, contributes to the secretion, distribution and drainage of the tear film. Several situations can affect and damage the anatomy of the lower eyelid, such as skin tumors, trauma and senile process. Reconstruction of the damaged lower eyelid is a challenge that aims at maximum functional and aesthetic preservation. Several eyelid reconstruction techniques have been described, from primary synthesis and skin grafting to the use of flaps, being necessary to consider, mainly, the extent of involvement of the lower eyelid and the depth (partial or total) when choosing the technique. The objective of this study was to report a case of lower eyelid reconstruction with Tripier myocutaneous flap and ipsilateral ear cartilage and perichondrium graft, a reproducible technique with good results. CASE REPORT: The clinical case report was carried out through medical record research in November 2025. The patient authorized the research through a free and informed consent form. The project was submitted to the Ethics Committee. DISCUSSION: The choice of technique for lower eyelid reconstruction should be made on an individual basis, considering mainly the extent of the defect, affected structures and patient profile. In the reported case, due to the involvement of the full thickness of the lower eyelid, the Tripier flap was used associated with a perichondrium and cartilage graft, with good evolution for the patient. CONCLUSION: The Tripier flap associated with the perichondrium and cartilage graft proved to be a good option for lower eyelid reconstruction with full thickness defect.
